Wellness INCENTIVE

Annual Wellness Incentive

Employees enrolled in an MLF medical plan may be eligible for a wellness reward of $225 by completing the wellness certification form by the required deadline. Please contact the Benefits Department or company nurses for the wellness certification form. All forms must be returned by December 3, 2027 for 2028.

Anthem Engagement rewards

You and your covered spouse may be able to earn rewards for participating in condition management, preventive care, Sydney engagement, claims-related activities, self-reported activities, and other wellness activities.

Rewards must be redeemed by December 31.

Track and redeem rewards through Sydney Health or Anthem.com.

Tobacco Cessation

Quit Now Indiana provides free, confidential tools to help employees, spouses, and dependents quit tobacco. Support may include a Quit Coach, Text2Quit, Web Coach, quitting aids if you qualify, and a quit guide.

Need an alternative way to qualify?

If it is unreasonably difficult due to a health factor to meet a wellness standard, or if it is medically inadvisable for you to attempt to meet the standard, contact the Benefits Department.

Maple Leaf Farms will work with you on another way to qualify where required by the program rules.
