Enroll through the UKG Benefit Admin System. Review your options, choose the benefits you want, and submit your elections before your enrollment deadline.
Your benefit start date depends on your employee group and eligibility rules. Check your benefits guide or UKG for the exact date that applies to you.
If you miss the deadline, you may have to wait until the next Open Enrollment period to enroll unless you have a qualifying life event.
Open Enrollment is the time each year when you can enroll in, change, or waive certain benefits for the next plan year. Elections made during Open Enrollment generally take effect January 1.
Eligible dependents may include your legal spouse and eligible children. You may be asked to provide documentation, such as a marriage certificate, birth certificate, or other proof of eligibility.
A spouse may be eligible, but the medical plan has a working spouse rule. If your spouse has medical coverage available through their own employer, they may not be eligible for Maple Leaf Farms medical coverage. Contact the Benefit Resource Center before adding a spouse.
Usually, you can only change benefits during the year if you have a qualifying life event, such as marriage, divorce, birth or adoption of a child, or a loss of other coverage. Report the change as soon as possible because deadlines may apply.
Look at the full cost, not just the paycheck deduction. Compare premiums, deductibles, copays, coinsurance, prescription costs, and the care you expect to use during the year.
A Health Savings Account, or HSA, lets eligible employees set aside pre-tax money for qualified medical expenses. HSA funds can roll over from year to year. You must meet IRS eligibility rules and be enrolled in an HSA-qualified medical plan to contribute.
A Flexible Spending Account, or FSA, lets you set aside pre-tax money for eligible expenses. Unlike an HSA, FSA funds generally must be used within the plan rules for that year, so choose your contribution carefully.
Use the right care option for your situation. Telemedicine or your regular doctor may work for many common issues, urgent care may help with non-emergency needs, and the emergency room should be used for serious or life-threatening situations.
Yes. Medical, dental, and vision are separate benefit elections. Review each option and decide which coverage you want for yourself and any eligible dependents.
Yes. Depending on your eligibility, Maple Leaf Farms may offer additional benefits such as an Employee Assistance Program, life and disability coverage, voluntary benefits, wellness resources, and a 401(k) plan.
Coverage may end when employment ends or when you no longer meet eligibility rules. You may be offered continuation coverage through COBRA when applicable.
