Documents & Notices

Important Documents & Legal Notices

Federal law requires MLF to provide certain notices. Key documents are summarized below. Contact benefits department for full copies or access them through the document center in the benefits admin system.

Summary of Benefits & Coverage (SBC)

A standardized document for each medical plan showing covered services, exclusions, cost-sharing, and coverage examples. Available from the benefits department.

Summary Plan Description (SPD)

The official governing document for each benefit plan. In any conflict between this guide and the SPD, the SPD governs.

Medicare Part D Notice

MLF's prescription drug coverage is considered Creditable Coverage — it meets or exceeds Medicare Part D standards. Keep this notice if you plan to enroll in Medicare later to avoid late enrollment penalties.

COBRA Notice

If you lose eligibility, you may continue coverage under COBRA (Consolidated Omnibus Budget Reconciliation Act of 1985). You will be notified of your rights and costs.

HIPAA Privacy Notice

The plan complies with HIPAA privacy requirements. Your personal health information is protected. A Notice of Privacy Practices is available from the benefits department.

Women's Health & Cancer Rights Act (WHCRA)

If you have or will have a mastectomy, you are entitled to coverage for reconstruction, prostheses, and treatment of physical complications, subject to plan deductibles and coinsurance.

Surprise Billing Protections

You are protected from balance billing for emergency services and certain out-of-network services at in-network facilities. You are never required to waive these protections. Questions? Contact CMS: 1-800-985-3059 or cms.gov/nosurprises.

Medicaid / CHIP Premium Assistance

If you or your dependents are eligible for Medicaid or CHIP, your state may help pay your employer plan premiums. Contact your state Medicaid office or call 1-877-KIDS-NOW. Special enrollment must be requested within 60 days of eligibility determination.
